Your Visit
You should plan to arrive at least 15 minutes before your appointment. Please remember to bring your:

- Insurance card(s). If you have a second insurance please bring both cards
- Current medications and dosages
- Medical records if you have them (especially immunization records)
- Referral from your physician (if required)

| You should do your best to keep your appointment. If a change is necessary, please call the Medical Center 24 hours before the time of your appointment whenever possible. Another patient may need your appointment. |
Registration
When you arrive you will speak to a registration clerk. He or she will ask you for your name, address, telephone number, social security number and insurance information.
Patients who have job related injuries and have filed a Workers' Compensation claim must bring a letter from their employer at the time of their visit, along with the name, address, and telephone number of their Workers' Compensation insurance company.
If your medical problems are from a motor vehicle accident, you need to bring the name, address, and telephone number of your auto insurance. You will also need a copy of the "Declaration Sheet" found in your motor vehicle policy. If your motor vehicle insurance company has paid all you are covered for, you need to bring a "Letter of Exhaustion" from your auto insurance company.
If you have retained an attorney for your medical problems, you need to provide the name, address, and telephone number of the attorney at the time of your visit.
Payment
We request payment at the time of your appointment for services which are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. For your convenience, Mastercard, Visa, Discover, American Express, personal checks, money orders and cash are accepted.
Obstetrical fees not covered by insurance are to be paid in full by the 8th month of pregnancy. Surgical fees will be discussed with you.
Patient Billing
You will be sent a single monthly statement for all services provided by the Medical Center. All balances are due in full upon receipt.
If you have any payment concerns or questions regarding your account, please contact Customer Service at (860) 348-4868 Monday through Friday from 8:00am to 4:30pm so a mutually beneficial arrangement can be made. Our staff will work closely with patients who require a payment plan.
Insurance Billing
Grove Hill Medical Center accepts Medicare, Blue Shield, and most major insurance plans. If you have provided your current insurance coverage information, we will submit a claim to your insurance company. You are responsible for all balances due after your insurance company pays its portion.
If you have any questions regarding which insurance plans we accept, please call our Customer Service line. Questions regarding your coverage and benefits should be directed to your employer or insurance company.
