Registration Patients who have job related injuries and have filed a Workers' Compensation claim must bring a letter from their employer at the time of their visit, along with the name, address, and telephone number of their Workers' Compensation insurance company. If your medical problems are from a motor vehicle accident, you need to bring the name, address, and telephone number of your auto insurance. You will also need a copy of the "Declaration Sheet" found in your motor vehicle policy. If your motor vehicle insurance company has paid all you are covered for, you need to bring a "Letter of Exhaustion" from your auto insurance company. If you have retained an attorney for your medical problems, you need to provide the name, address, and telephone number of the attorney at the time of your visit. Payment Obstetrical fees not covered by insurance are to be paid in full by the 8th month of pregnancy. Surgical fees will be discussed with you. Patient Billing If you have any payment concerns or questions regarding your account, please contact Customer Service at (860) 348-4868 Monday through Friday from 8:00am to 4:30pm so a mutually beneficial arrangement can be made.
